오늘날 우리가 경험하고 있는 covid-19와 발생빈도가 점점 커지고 있는 자연재해에 직면하면서 의료기관에 요구되는 재난대비계획을 살펴보기 위하여 미국과 일본, 그리고 우리나라를 중심으로 재난관리제도의 틀 속에서 재난대비계획의 구조를 살펴보고, 우리나라가 재난대비계획을 개선할 때 참고할 필요가 있는 내용을 도출하였다. 연구방법으로는 미국 의료기관 재난관리의 틀과 위험평가도구 UCLA hHAP, Kaizer HVA, PHRAT를 조사하고, 일본 의료기관 MBCP, 한국 의료기관 NIPP를 조사하였다. 연구결과로는 첫째, 의료기관이 재난을 예방하고 대비하도록 하는 위험평가는 주요거점병원뿐만 아니라 일정규모 이상의 입원환자를 보살피는 일반병원에서도 업무중단에도 불구하고 환자의 건강을 유지할 수 있도록 개선 및 활용될 필요가 있다. 둘째, 미국에서는 민간 의료기관인증원이나 의료재단에서 재난대비계획을 위한 위험평가도구를 개발하여 실용화하고, 공공의료부서에서도 지역사회의 공공의료서비스를 통합한 위험평가도구를 개발하여 의료기관을 지원하고 있다. 그리고 일본에서는 후생노동성에서 업무연속성계획의 지침과 수립, 그리고 교육을 지원하고 있다. 셋째, 재난대비계획을 위해 활용할 수 있는 위험평가 도구는 평상시와 재해시로 구분하여 의료기관의 인력상해, 경영자원손실, 업무연속성에 대해 계획을 수립할 필요가 있고, 나아가 병원 원내뿐만 아니라 지역사회와 연계하여 재난대비 역량을 강화하는 것이 필요하다. 넷째, 미국의 의료기관이 활용하는 위험평가도구는 기초데이타를 입력하면 자동으로 점수화되고 정량화되는 엑셀파일 템플릿으로 제공되고 있어 사용이 용이하다. 일본 및 우리나라의 경우 세부적으로 문서화되므로 기준의 간소화, 지침개발 및 문서작성의 지원 등의 측면에서 개선방안이 요구된다. 우리나라 의료기관은 그동안 환자의 안전사고를 방지하기 위한 안전강화 역량을 높이는데 주력해 왔다. 그러나 이제는 재난관리와 재난대비계획을 수립하는데 적합한 접근방안을 찾도록 노력해야 한다. |

When disaster strikes, medical institutions play a vital role in protecting the health and life of patients. Prior to a catastrophe, it is necessary
to establish a disaster mitigation activity plan to effectively respond during an onset and to quickly secure business continuity to prevent
irreversible damage. This study aims to provide direction in the event of a disaster pertaining to risk assessments and business continuity
plans by comparing the current tools and guidelines of medical institutions in the United States, Japan, and Korea. Among the hazard
vulnerability assessments conducted by medical institutions in the United States, the Hazard Risk Assessment Instrument, Hazard and
Vulnerability Analysis by Kaizer Permanente, and the Pennsylvania Public Health Risk Assessment Tool were examined. The medical Business
Continuity Plan in Japan, and the National Infrastructure Protection Plan in Korea were also analyzed. The results of this research indicated
that according to the Disaster and Safety Act, the preparedness plan should include medical institutions, not just emergency medical services.
It is essential for risk vulnerability assessments and business continuity plans to seek varying tactics and approaches. Additionally, major risks
and key management risks should be prepared in advance and assessed through data accumulation. |
