(연구배경 및 목적) 최근 급속한 고령화와 고령가구 증가로 그들의 건강한 삶을 지원하기 위해 정부에서는 커뮤니티케어를 도입하였고, 이를 실현하기 위한 지역사회 내 커뮤니티케어시설들의 공간 및 운영계획이 필요하다. 이에 본 연구에서는 커뮤니티케어의 효시인 일본 지역포괄케어시설의 공간 및 운영 특성을 살펴봄으로써 향후 국내 커뮤니티케어시설의 효율적 공간 및 서비스 운영방안 모색을 목적으로 진행되었다. (연구방법) 첫째, 사례연구대상은 일본오사카시(大阪市) 히가시나리구(東成?) 지역포괄케어시설의 고령자종합복지센터에 있는 주간보호센터, 단기보호시설, 요양시설 등과 지역밀착서비스센터에 있는 북부지역포괄케어센터와 생활지원주택 등을 대상으로 조사를 진행하였다. 둘째, 현장조사는 공간구성 및 디자인 특성을 위한 사진촬영과 담당자와의 질의응답을 통해 조사하였다. 셋째, 심층면접은 시설운영자와 지역사회협의회 운영자 2인을 대상으로 공간 및 운영 특성에 대해 약 2시간가량 인터뷰를 진행하여 내용분석법을 활용하여 분석하였다. (연구결과) 사례조사대상은 지역의 도로체계를 고려하여 입지하고 있으며, 대중교통의 접근성이 높은 곳에 위치함으로써 고령자의 도보생활권과 접근성을 고려하고 있는 것을 볼 수 있다. 공간구성특성으로는 중앙의 거실 및 식당 또는 프로그램실 등 공용공간을 배치하고 개인실 및 직원실을 병렬로 배치하여 공용공간의 이용활성화와 입소자 및 직원간의 커뮤니케이션의 기회를 확대하고 있다. 공간디자인에서는 필로티구조를 통한 안전성확보, 핸드레일설치 및 문턱제거 등의 베리어프리디자인 적용, 휠체어 이용자를 고려한 가구 및 도어디자인을 통해 신체적 변화특성에 대응하고 있으며, 다양한 칼라를 적용하여 고령자의 인지를 높이며 정신적 변화특성에 대응하고 있고, 중앙에 변용 가능한 가구를 배치하여 사회적 활동을 지원하고 다양한 소파 및 의자의 배치를 통해 사회적 활동 및 교류를 지원하여 사회적 변화특성에 대응하고 있다. 입소자의 편리한 시설 이용과 함께 지역사회와 함께 하기 위한 다양한 협력 프로그램을 운영하고 있다. (결론) 고령자의 건강한 삶을 실현하기 위해서는 지역사회 내 계속거주를 지원할 수 있는 공간 및 운영방안이 필요함을 확인할 수 있으며, 이러한 시설 이용에 있어 노화로 인한 신체적, 정신적, 사회적 특성의 변화에 대응할 수 있는 공간구성과 디자인의 적용이 필요하다. 고령자 커뮤니티케어시설이 지역사회 내 고립된 섬으로 존재하는 것이 아닌 지역사회 다양한 자원 및 조직과의 협력과 연계를 통해 지역과 함께할 수 있는 운영방안이 매우 중요하며, 이는 고령자들의 지역사회 내 계속거주를 실현하여 건강한 삶의 지원토대가 될 것이다. |

(Background and Purpose) This study examined the characteristics of space and operation of the comprehensive care facilities in Japan, which is a pioneer of community care, and searches for efficient ways of space and service operation for the domestic community care facilities. (Method) First, the case study subject included the daycare center, short-term caring facility, and nursing facility in the comprehensive welfare center for the aged and the northern region comprehensive care center and life support housing in the community-oriented service center of the regional comprehensive care facility located in Higashinar-ku, Osaka, Japan. Second, field survey was conducted by shooting photographs that capture the space composition and design characteristics of the facilities and interviewing the person in charge. Third, in-depth interview was conducted with an administrator of the facility and a community committee administrator for about two hours and a content analysis method was used to analyze the interview results. (Results) The case subject was built by considering the road system of the region and walking distance and accessibility of the aged with high accessibility from public transportation. As for the space composition characteristics, communal space, such as living room, dining room, and program room, was located at the center and individual room and staff room were placed in a row for the purpose of activating the use of the communal space and expanding an opportunity of communication among the residents and staffs. The spatial design was well reflecting the elderly people's characteristics that change following the aging. For physical changes, safety was secured through piloti structure, barrier-free design, such as installation of hand rail and removal of door sill, was applied, and furniture and door was designed to accommodate wheelchair users. Diverse colors were used in the design to increase the elderly residents' recognition, which is one of the considerations for the psychological changes. For changes of social characteristics, changeable furniture was installed at the center for supporting diverse social activities and various allocations of sofa and chairs allowed social activity and exchange among the residents. As for the operation, diverse collaborative programs aimed at connecting with the local community were implemented, in addition to the residents' convenient use of facilities. (Conclusions) This study confirmed the necessity of a space and operation plan that can support aging in community for the realization of healthy life of elderly people. When building the facilities, an application of space construction and design that can respond to the elderly people's physical, psychological, and social characteristics that changes due to aging is necessary. Instead of being an isolated island inside a local society, the community care facilities for the aged should be run together with the region through collaboration and connection with diverse resources and organizations of the local community. |
