장기요양보험제도의 시행에 따라 사회로부터 노인요양시설은 운영상의 다양한 변화를 요구받고 있지만 요양보호서비스의 질적 수준, 인력배치, 재원조달, 효율적인 지원관리 체계 등의 변화가 아직 미비한 실정이다. 이에 연속보호체계형 은퇴단지(CCRC) 내 노인요양시설의 효율적인 관리운영을 위한 개선방안을 제시할 필요가 있다. 이를 위해 본 연구에서는 CCRC 내 노인요양시설의 관리운영의 실태를 분석하고 그 특성을 규명하고자 한다. 이 연구의 조사대상은 CCRC의 주거유형 중 노인요양시설로서 2018년 기준 우리나라의 보건복지부에 공식적으로 등록한 연속보호체계형 은퇴주거단지(CCRC) 내 노인요양시설로 제한하였다. 조사방법으로 문헌조사와 전문가 자문을 통해 노인요양시설의 관리운영 특성에 대한 조사항목을 도출하여 개발하였다. 이를 활용하여 노인요양시설의 관리운영 전문가인 시설장이나 실무담당자를 대상으로 면접조사를 실시하였다. 단 환경 및 시설관리 분야의 시설 및 설비관리 중 환경조성 항목은 실측조사를 하였다. 조사기간은 2019년 1월 17일부터 2월 22일까지 이루어졌다. 조사내용은 노인요양시설의 운영관리 특성을 기관관리, 인적자원관리, 환경·시설관리, 거주자 돌봄관리, 재무관리, 지역사회자원 활용, 마케팅 등 7개 분야로 분류하였고, 아울러 각 시설의 관리운영 분야 중 우수한 분야 및 개선이 필요한 분야와 그 이유와 노인요양시설과 노인복지주택 간의 연계성에 대한 내용을 포함하였다.
조사 분석 결과 다음과 같이 결론을 내린다. 1) 한국의 노인요양시설의 관리운영특성 중 거주자 돌봄관리와 인적자원관리 분야가 비교적 우수하고, 지역사회자원 활용과 재무관리, 환경· 시설관리이 개선이 필요하다. 관리운영 특성 중 제일 취약한 분야는 지역사회자원 활용인데, 요양시설의 입소노인의 건강상태로 인하여 거동하기 어렵고 외부인과의 접촉을 꺼리기 때문이다. 환경·시설관리의 부족한 부분은 시설규모가 작은 경우 내부의 공용공간의 협소함이 그 원인이었다. 2) 일반시설은 장기요양기관에 비하여 직원이직률이 낮았고, 입소자 1인당 연 면적이 두 배 정도로 넓었으며, 시설설비의 안전성·쾌적성 수준이 높았고, 내부 공용공간의 종류가 다양해서 전체적으로 환경 및 시설설비가 우수하였다. 3) CCRC의 특성을 보이는 노인요양시설과 노인복지주택 간의 연계성 측면을 보면, 노인요양시설 입소자는 동일한 은퇴단지 내에 있는 노인복지주택의 정원 및 산책로 등 옥외공간과 실내의 강당, 식당, 로비, 종교시설, 사우나 등의 공용공간을 이용할 수 있다. 이 연구는 연속보호체계형 은퇴주거단지(CCRC) 내 노인요양시설로 그 대상을 제한하였기 때문에 전체 노인요양시설의 관리운영특성에 대한 분석이 아니라는 제한점이 있다 |

After the long-term care insurance policy is implemented, a nursing home in continuing care retirement community (CCRC), is mostly not adequate for quality level of nursing care service, manpower placement, financing and efficient support management system. Therefore, we need to investigate the status of nursing home, and find the way to improve it. The purpose of this study is to investigate the conditions of nursing home and to define its management characteristics. Using the investigating tool, we conducted a survey on workers in nursing home in January and February 2019. The findings from this are as follow: In large, Korea’s nursing home showed relatively good management on human resources and resident’s care, whereas showed relatively bad management on community resources utilization, financial and environment · facility. Residents living in nursing homes within the CCRC may use common spaces inside and outside of welfare house for the aged in the same complex. In addition, if the health status of the couple is different among the residents living in the CCRC, the healthy ones of the couples live in the welfare house for the aged, and the less healthy ones live in the nursing homes in the same complex. This study will be used as a fundamental data in planning a nursing home of CCRC. |
